The packages provide individual case management and funding to purchase additional support and services to meet the needs of each person. Case management services work with the person receiving the packaged care to ensure that those services are meeting their need and that they are timely and appropriate.
Our In Home packages service the region of South West Victoria. The catchment area includes from Princetown to the South Australian Border as far North as Skipton, and Balmoral. Recipients of these packages require an assessment from the Aged Care Assessment Service (ACAS). People accepted onto the programs are generally 70+ years of age, or for people from aboriginal communities, 45+ years.
