A Community Aged Care Package for Kooris and Aboriginals and Torres Strait Islanders (CACP-ATSI) is an Australian Government funded program that provides funding specifically to assist frail older people from Aboriginal communities to continue living in their community.
The program provides a package of care which includes a case manager to advocate for the client and coordinate services to meet their care needs. Care needs are identified and tailored to meet their circumstances within the available resources and funding. Services may include home care, personal and respite care, recreational activities and equipment, and social support. Case Managers will work together with the client and their family and carers to identify care needs and develop a plan to maximise their independence.
For people from Aboriginal Communities to be eligible for a CACP - Koori and ATSI package, individuals must be referred to the Aged Care Assessment Service for an assessment for eligibility for low care community services. Eligibility criteria also includes the following:
As with all community service providers, the CACP-ATSI program requires a client contribution in the form of a negotiated fee.
