A Community Aged Care Package (CACP) is an Australian Government funded program that provides funding to assist frail older people to continue living in their community.
The program provides a package of care which includes a Case Manager to advocate for you and coordinate services to meet your support needs. Care needs are identified and tailored specifically for the clients within available resources and funding. Services may include home care, personal and respite care, recreational activities, sourcing of equipment, and social support. Case Managers will work together with clients and their family and carers to identify their care needs and develop a plan to maximise their independence.
To be eligible for a CACP, individuals must be referred to the Aged Care Assessment Service for an assessment for eligibility for low care community services. Eligibility criteria also includes the following:
