Funded, flexible service delivery for carers of a person living with dementia

In Home Respite is a Commonwealth Government funded initiative through the National Respite for Carers Program.  The program enables carers of people living with dementia to receive funded support and service coordination.  
The program is designed to assist carers in their caring role and to help the carer to continue living independently in their own home with appropriate care and support. 
The individual needs of carers and care recipients are met by providing:
  • personal hygiene support
  • home help services
  • day and after hours respite care
  • shopping support
  • social participation
  • transport
Respite care funded by the program can be delivered by our own care staff and/or purchased from personal care agencies qualified to meet the unique needs of people with dementia.  Services can be offered on a continuous or casual basis, during the day or overnight.
Carers of people living with dementia living in the South Western Region of Victoria are eligible to apply for this program and referrals are accepted from any source, including the client themself, their carer, doctor, or other health service providers.  If preferred, referrals may be directed to us using the Service Coordination Tool Template (SCTT).  Each referral is considered and prioritised on the basis of need.
Upon acceptance to the program a small client contribution will be negotiated in the form of a weekly fee.
