Make sense of your care options

Recent illness, disability, bereavement or simply the passing of the years may make it harder to manage at home without help. When you think you are no longer able to manage at home without assistance or that this situation is imminent – what can you do?

When looking into care for the first time, you will be presented will a great deal of information from many different organisations such as hospitals, government agencies and care providers. Navigating your way through the entitlements, acronyms and other rhetoric can be overwhelming, particularly when care is being sought as a result of unforeseen circumstances.

The best place to start is by talking to your doctor. If they agree that you require some level of care assistance they will refer you to the Aged Care Assessment Service or ACAS.

The ACAS is a team of health professionals who help older people and their carers work out what kind of care will best meet their individual needs. The ACAS serving Warrnambool and surrounding districts, known as South West ACAS, is based here on the Lyndoch Living site. Once you have made an appointment with them, a member of the South West ACAS team will visit you, in your home or in hospital, to make an assessment of your needs.

This assessment is required to make you eligible for a selection of services provided by Lyndoch Living. So once the assessment has been completed, we recommend contacting us so we can guide you through our services and what we can do for you.
