Specialist support to assist people to overcome difficulties in their lives

Lyndoch Living provides specialist services for people who have a diagnosed Acquired Brain Injury (ABI).  The programs offer a flexible response, and individually tailored case management to maximise client outcomes. The programs target two distinct groups:

  • People with an Acquired Brain Injury as a result of alcohol and other drugs.
  • People with an Acquired Brain Injury that is non- compensable and who have complex issues requiring assistance through case management.

Both programs provide a Case Manager who will work with the individual to identify personal goals and assist in providing appropriate support mechanisms to help work towards those goals. The Case Manager can also assist with the coordination and purchase of support services. Within a three month time frame, the program works toward a planned discharge from the program when the agreed goals have been met.

Acquired Brain Injury Program

To be eligible for this program, clients are required to be:

  • Diagnosed with an  Acquired Brain Injury (or a presumed diagnosis)
  • A voluntary participant
  • A resident of the Barwon South Western region
  • Aged from 18 to 64 years
  • Ineligible for any compensation payments which provide case management (eg. Workcover, TAC)
  • Not receiving other case management services


Acquired Brain Injury - Alcohol and Other Drug (AOD) Program

To be eligible for this program, clients are required to:

  • Be diagnosed with an  Acquired Brain Injury (or a presumed diagnosis) as a result of alcohol and other drug use
  • Have an Acquired Brain Injury and an Alcohol or other substance dependence
  • Be a voluntary participant
  • Be a resident of the Barwon South Western region
  • Be aged from 18 to 64 years
  • Be ineligible for any compensation payments which provide case management (eg. Workcover, TAC)
  • Have undertaken a neuropsychological assessment
  • Be a registered client with an alcohol and drug support service
  • Be willing to undertake ongoing alcohol and other drug counselling
  • Not be receiving other case management services

These programs help people to become integrated into their community by identifying and sourcing the specialist support services they need which will reduce the impact of their disability on their daily life.

To access the ABI programs, referrals are accepted from any source, including the client themself, their carer, doctor, alcohol and drug counselling agencies or other health service providers.  If preferred, referrals may be directed to us using the Service Coordination Tool Template (SCTT).
