Supporting people with a disability to live independently

Lyndoch Living provides case management on behalf of the Department of Human Services for people with a disability. This service is only available for people who receive funding through an Individual Support Package and who are registered clients with Disability Services Victoria. People who receive funding under this program and have complex care needs can receive case management support to assist them to manage the care they receive.

Case management provides a key worker who will be the client's single point of contact for any issues relating to the coordination of care and who will advocate on the client's behalf to ensure they are provided with appropriate care and support.
