Strengthening independence in a supported residential setting

The Supported Residential Service (SRS) program provides services to assist people in living in supported accommodation with their daily activities.

To be eligible for this program clients must be a resident of a registered Supported Residential Service in the South West Region who:

  • Is in receipt of a pension
  • Has identified needs not being met by mainstream services
  • Exhibits challenging behaviour
  • Is not eligible for any compensation payments which provide case management (eg. Workcover, TAC)
  • Is not eligible for any other case management program
  • Is not a registered Disability Services client
  • Has not recently been discharged from a psychiatric service

Lyndoch Living can provide a Case Manager for SRS residents for up to two hours per week per person, to assess your needs and develop a service plan in conjunction with your carers and family and any other service providers involved.

The service delivered can assist you by:

  • Maintaining lifestyle within the chosen environment
  • Creating opportunities for socialisation and recreation activities
  • Giving budgeting advice

To access the SRS program referrals are accepted from any source, including the client themself, their carer, doctor, or other health service providers.  If preferred, referrals may be directed to us using the Service Coordination Tool Template (SCTT).
