In-home services

Are you not quite ready for residential living, but still need a hand with some odd jobs around the house? Lyndoch Living provides several packages to maintain your independence and allow you to continue living at home.

After being allocated a support advisor through a home care package, you can be linked to a wide range of different services including, personal care, respite care, clinical care, transportation to appointments, social support and health aids and equipment. Your support advisor will help you identify your needs and choose the type of care you need and the care provider. 

The Home Care packages is an Australian Government funded program however it also requires a client contribution. Our Warrnambool aged care support advisors know which programs and services are available to support you and how they are funded.

Supported Residential Service

The Supported Residential Service (SRS) program provides services to assist people with their daily activities, living in supported accommodation. A case manager can be provided for up to two hours per person per week.

To be eligible for this program, clients must be a resident of a registered Supported Residential Service in the South West Region. They must also:

  • be in receipt of a pension
  • identify that their needs are not being met by mainstream services
  • exhibit challenging behaviour
  • not be eligible for compensation payments that provide case management – e.g. Workcover or TAC
  • not be eligible for any other case management program
  • not be registered as a disability services client
  • have not recently been discharged from a psychiatric service

The client may refer him or herself, or receive a referral from their carer, doctor, or other health service providers. Referrals may be directed to us using the Service Coordination Tool Template (SCTT).
