A great career opportunity exists for an Intake and Assessment Officer to join our organisation for a 12 month fixed term full time temporary position. Lyndoch Living provides a diverse range of care, accommodation, community based and in home service options to the community of South West Victoria.

Reporting to the Homecare Operations Manager, the role will provide the initial point of contact for individuals seeking services from Lyndoch Living as well as coordinate referrals to ensure they are directed to the appropriate programs with the appropriate information. Demonstrated experience working in Home Care Packages, Community Home Support Programs and/or My Aged Care will be highly regarded.

The position will also manage a client case load.

The successful candidate will need to be proactive in the ongoing development of the role which will include collaboration with other services across Lyndoch Living.

Our recruitment procedures reflect our commitment to the safety and protection of our clients, relevant screening including national police checks will be conducted. Applicants who have lived overseas for 12 months or longer during the past 10 years are required to provide the results of an international police check.

You will also be subject to a Disability Worker Exclusion Scheme check. This is a check of an individual’s name against a DHHS database that records individuals who pose a proven risk to the health, safety or welfare of people with a disability. If your name is on the list, you will be ineligible to work at Lyndoch Living.
