Lyndoch Living’s in-home services are not only for the elderly. Support services for those with a disability or complex care needs, irrespective of age, are also available. We offer three services which are flexible and adapted to support clients who wish to remain living in their own homes in Warrnambool and the surrounding districts of Hamilton and Portland.
The Linkages Program
The Linkages program supports people of any age who have complex care needs due to a health condition or disability and are therefore vulnerable of premature or early entry into a residential care facility.
The program provides a case manager to work with the individual and their family/carers to identify support needs, act as an advocate, monitor the client’s ongoing needs, and develop a plan to maximise the client’s independence within the available resources and funding.
To be eligible for the Linkages program, clients are required to reside in one of the five municipalities and possess:
Funding from the Commonwealth and Victorian Governments under the HACC program supports the program. The client may refer him or herself, or receive a referral from their carer, doctor, or other health service providers. Referrals may be directed to us using the Service Coordination Tool Template (SCTT). Carers can also contact Lyndoch to discuss our respite care services in Warrnambool.
Acquired Brain Injury Program
Lyndoch Living provides specialist services for people who have an Acquired Brain Injury (ABI) as a result of alcohol and other drugs, or is non-compensable resulting in complex issues requiring case management assistance.
Both programs provide a case manager who will work with the individual to identify personal goals and assist in helping them work towards those goals. The program follows a three-month time frame, with the aim to achieve the client’s goals followed by a planned discharge from the program.

These programs help people to integrate into their community and reduce the impact of their disability on their daily life with the specialist support services required.

Individual Support Package
Independent living support package for those living with a disability is only available for people who receive funding through an Individual Support Package and who are registered clients with Disability Services Victoria.
Lyndoch Living will provide a case management key worker on behalf of the Department of Human Services for people with a disability. They will act as the client’s single point of contact and advocate on the client’s behalf for any issues relating to the coordination and provision of appropriate care and support.
